A falling injury rate, passed audits, up-to-date procedures: everything looks under control. Yet many organisations discover after a serious accident that the barrier meant to prevent it had not been in place for a long time. Compliance describes what should happen; control proves what actually happens.
Five questions for your next leadership meeting
- What are our top ten critical risks? The ones that can kill or seriously injure, site by site, each with a named owner.
- Which critical controls hold them? Not the list of procedures: the few barriers whose absence makes the accident likely.
- When were they last verified in the field? By whom, with what result, and what was done about the gaps?
- What do our pSIFs tell us? Are potentially serious events reported, analysed and shared?
- Are our actions effective, or just closed? An action closed in a tool is not a reduced risk.
Moving from intentions to evidence
If these questions are met with silence, that is normal: they address what traditional indicators do not show. The good news is that it takes few elements to answer them — a critical-risk map, defined controls, a verification routine and a dashboard focused on weak signals.
The BETTERISE Peace of Mind Assessment asks these questions in ten points and gives you a first reading in five minutes.
